- Company Name: What is the company name on the card.
- Address 1: What is the address line 1 on the card.
- Address 2: What is the address line 2 on the card.
- Country: What is the country on the card.
- Postal Code: What is the Postal/Zip Code on the card.
- City: What is the city on the card.
- State: What is the state on the card.
- Card Number: What is the card number.
- Other Number: What other numbers exist on the card.
- Expiration Date: What is the expiration date on the card.
- CVV Code: What is the CVV Code on the card.
- Classification: What classification is on the card.
- Restrictions: What restrictions exist for the user on the card.
- Height: What is the person's height on the card.
- Weight: What is the person's weight on the card.
- Birthday: What is this person's birthday on the card.
- Place of Birth: Where was this person born.
- Start Date: When was this card received
- Note: If this information is blank, there is no start date.
- End Date: When does this card expire.
- Note: If this information is blank, there is no expiration date.
- Display Order: This is the order in which the URL link is displayed publicly as well as within the grid. The lower the number, the closer to the top it shows.
- Notes: Any notes that you have related to the URL link.
- Active: Is this URL link active to be displayed or used within the system.
- Is Protected: Is the information private and shouldn't be shared to regular users. This information is also double encrypted for added security.
- Is Donor: Is the person a donor
- Card Image Front: This would be the front side of the card that you have scanned into the computer.
- Card Image Back: This would be the back side of the card that you have scanned into the computer.
